      Well, 8-Bit Theatre did break the fourth wall by Black Mage having a strategy guide on the game he is in, looking up the Giant's HP and said it has a 124 HP, a brrom talkign backwards and how four white mages as aparty could never work... until much later he was proved wrong.

      No-one said Ginny couldn't help but wander off for perhaps the toilet or something, not even Ron would watch his siter THAT closely, plus Kazari can manpuilate his appearance as a Greeed.

      Besides, Ichigo's actions are kinda justified, when he used Bankai, he did shatter a dimension that kept him and his enemy inside.

      Your guess is as good as mine, I suppose it's because Kon had been in Ichigo's body more than once and some leftor reiastu was placed in Kon or maybe, Kon really did forget and just bragged that he never once forgot Rukia as he never personally admits to anyone about what he feels.
