A double idiom. Depending on how it's used, it could mean either other people in straight jackets or people who work for the state mental health hospital. They're often portrayed wearing white coats. Here, it implies the latter.

 

What's great, is that during this entire song.... he's talking to his dog...

 

Actually, he prefers it when I sing. He gets angry when he stops hearing the melody to "It's raining Men" for some reason.

Haha, I remember this song. I use to sing it all of the time and people would look at me like I was nuts...(Sigh) Good times, good times...

 

I don't sing it and people still look at me like I'm nuts. I ask my imaginary guitar playing grasshopper friend why, but he just says "Whatever Queer."
